Forms to Complete Your Financial Aid Application
In some cases, we need additional documents to complete your financial aid application and provide a full and accurate financial aid package. We’ll let you know when that happens and can answer any questions you may have.
You can upload requested documents to the College Board's IDOC portal or through Whitman’s secure ShareFile link.
FAFSA Verification Forms
When you submit the Free Application for Federal Student Aid (FAFSA), the Department of Education may randomly select your application for a process called verification. This will be noted in your Student Aid Report (SAR). If you’re selected, Whitman College is required to collect additional documents from you and your family to confirm the accuracy of the information on your FAFSA. Required documents may include:
- Dependent Verification Worksheet - V1: Confirms household and tax information for students who are considered dependents on their FAFSA.
- Independent Verification Worksheet - V1: Confirms household and tax information for students who are considered independent on their FAFSA.
- Verification Worksheet - V4: Verifies identity and high school completion status for selected students.
- Dependent Verification Worksheet - V5: Combines V1 and V4 requirements to confirm household, tax information, identity, and high school completion status for dependent students.
- Independent Verification Worksheet - V5: Combines V1 and V4 requirements to confirm household, tax information, identity, and high school completion status for independent students.
Whitman-Specific Forms
- Outside Resources/Outside Scholarships Form: Reports any outside scholarships, grants or other resources that will help pay for your education.
- CSS Profile Waiver Request for the Noncustodial Parent: Allows a student to request that the noncustodial parent’s financial information be excluded from financial aid consideration due to special circumstances.
- Household Verification Worksheet: Confirms the number of people in your household and their relationship to you for financial aid purposes in cases where there is a discrepancy between the information provided on the FAFSA and CSS Profile.
- Asset Verification Worksheet: Confirms the value of assets in cases where there is a discrepancy between the information reported on the FAFSA and CSS Profile.
Additional Financial Aid Forms
Depending on your circumstances, you or your family may need these additional forms:
- Request Form for Computer Purchase Budget Adjustment: One time during college you may request an increase in your cost of attendance to include the purchase of a computer, which may make it possible for you to receive additional outside scholarship funding or increase your loan or work-study amount.
- Parent PLUS Loan: Enables parents of dependent students to apply for a federal loan to help cover educational costs.
- Provisional Independent Status Clarification Form: Provides information to determine if you qualify for independent status on your FAFSA.
- Request for 9th Semester of Financial Aid: Allows students to request an additional semester of financial aid.
