Spring 2022 COVID-19 Requirements and Forms

Thank you for your ongoing cooperation with our campus COVID-19 protocols. Please review the information below and complete the requirements and appropriate forms by the necessary deadlines.

1. Pre-return testing: all students must take a COVID-19 test 24 to 72 hours before beginning travel to Walla Walla and complete the Spring 2022 Pre-Arrival COVID Test form by January 15, 2022.
2. Provide proof of COVID-19 vaccine initial doses: every member of the Whitman community is required to have received two doses of the Pfizer, Moderna, or other WHO-approved two-dose vaccine or one dose of the Johnson&Johnson/Janssen vaccine, or have an approved medical or religious exemption. New employees, students and volunteers must provide proof of vaccination by January 15, 2022.
   • To provide proof of vaccination complete the COVID-19 Vaccination Reporting Form.
   • If you wish to apply for an exemption, please submit a letter to the COVID-19 Vaccination Exemption form.
3. Provide proof of COVID-19 Booster: every eligible member of the Whitman community is required to receive the COVID-19 vaccine booster or have an approved exemption by January 15, 2022.
   • If you are already eligible for the COVID-19 vaccine booster (at least six months after your second dose of Pfizer, Moderna or another two-dose vaccine or two months after your first dose of Johnson & Johnson), you must receive the booster by January 15, 2022. Fill out the COVID-19 Vaccination Booster - Completed form after you have received the booster.
   • If you are not yet eligible for the COVID-19 vaccine booster (it has been less than six months since your second dose of Pfizer, Moderna or another two-dose vaccine or less than two months since your first dose of Johnson & Johnson), you must fill out the COVID-19 Vaccination Booster - Pending form by January 15, 2022, providing the date that you become eligible and attesting that you will receive the booster within one month of that date in order to maintain your campus access. 
   • If you already have a medical or religious exemption for the COVID-19 vaccine, you do not need to do anything. You will be granted a corresponding exemption to the booster requirement.
   • If you wish to apply for an exemption due to the booster’s emergency use authorization (EUA) status, you should submit a letter to the COVID-19 Vaccination Exemption form stating that you intend to wait to receive the booster until any one of the booster vaccines has received regular use authorization from the FDA. Anyone granted this exemption will be required to receive a booster dose within one month of when the EUA has been lifted for any of the available COVID-19 vaccines.
4. Baseline COVID-19 Testing: Every staff, faculty, student, retiree and volunteer with campus access must be tested twice during the baseline testing period: as soon as possible after returning to campus/Walla Walla and a second test 5–10 days later.

   All baseline testing will take place at Cordiner. The baseline testing schedule will be as follows:

   • January 4–7 and 10–12: 11 a.m. to 1 p.m.

   • January 13 & 14, 17–21, 24–28: 8 a.m. to 4 p.m.

   You must take your first test by January 21 in order to complete your second test by the close of testing on January 28. There will be no make-up testing outside of the currently scheduled time. If you do not complete two tests at least five days apart by January 28 your campus access will be suspended.
