Approval Documents

If you have been approved for spring 2009 Study Abroad Leave by Whitman College, you will receive notification in your campus mailbox no later than noon on Thursday, October 9th. Please read carefully all of the documents in your approval packet.

Congratulations! You are about to embark on one of the most rewarding adventures of your life. With Study Abroad Leave Approval from Whitman College, comes a few more required steps. The following forms need to be downloaded, completed and returned to the Study Abroad Office no later than Tuesday, November 11th. Also, on Saturday, December 13th from 1pm to 4pm in Olin 130, each study abroad participant will attend the required Pre-Departure Study Abroad Orientation. If you have a class-related conflict (i.e. a required field trip) on that date, you should inform the Study Abroad Office immediately.

Health Statement and Liability Waiver Form
(All Programs)

• It is every student’s responsibility to carefully read and become familiar with the health and safety issues of their study abroad destination and the demands associated with studying abroad. Please select your study abroad destination from the lists of countries on the following websites Center for Disease Control (CDC) and the U.S. Department of State and read through carefully.

Proof of Medical Insurance Form
(Affiliated Programs)
• All students who attend an affiliated program will be required to have health insurance. If you have insurance coverage other than the Whitman Student Group policy, check with your insurance company to ensure that you will be covered both in the U.S. and abroad.

ISIC Card Application
(Affiliated Programs)

• All students who attend an affiliated program will be required to purchase an ISIC card through the Study Abroad Office. An ISIC card offers emergency evacuation insurance, repatriation, discounts on airfare, and much more. If you are planning to travel outside of your program dates, it is recommended that you purchase an ISIC card.

Power of Attorney Form
(Optional - All Programs)

• All students who receive need-based financial aid are highly recommended to complete this form giving your parents (or another designee) power of attorney for signing documents in your absence. This will help you get loan checks processed quickly. Once you complete the document, you may stop by Mem. 216 or 102 to have it notarized.
