Reporting an Incident of Sexual Misconduct

The purpose of the Sexual Misconduct Incident Report Form is two-fold: 1)to allow the College to track which services are being accessed and to assist the College in doing a more effective job of educating students with regard to the services available for dealing with sexual misconduct; 2)to allow the College to report more accurately the number of sexual misconduct incidents occurring on campus each year in compliance with the "Clery Act".

The Sexual Misconduct Incident Report Form cannot be introduced in conjunction with any of the procedures specified in Whitman's Sexual Misconduct Policy; initiation of these procedures requires that the person involved contact either the Judicial Coordinator or the Dean of Students.

Sexual Misconduct Incident Report Forms will be maintained in a secure file for seven years from the date filed, at which time they will be destroyed.

This is a very important form and should be filled out as accurately and honestly as possible.  Your assistance in completing this form is much appreciated.  Please note that, in the absence of a reporter's name on the front side of the sheet, this form cannot accomplish the above objectives.
