Records required for enrollment

Required information must be completed and returned to Welty Health Center by July 10.

• Treatment Authorization Form signed by parent/guardian (for all students under age 18)
• Proof of health insurance coverage or purchases of the college health insurance policy (See CLEo)
• Comprehensive Student health and history form

Failure to produce health forms results in a registration hold!

All medical records are strictly confidential and are not part of the college academic record system.

No medical information can be released without written permission of the student.
