The Welty Center, Student Health Services staff extends to you a warm welcome. We are here to offer assistance with your health concerns and needs. Whitman College believes that students pursuing a healthy lifestyle receive the optimum benefits from their liberal arts educational experience.

Our Health Center is open 24 hours per day for all full-time students during the academic year regardless of type of insurance coverage. A physician is available every weekday morning between 8 and 10 a.m. to see students at no charge. The director of the Health Center, a family nurse practitioner, is also available to see students on an appointment basis during the week. A nurse offering care and advice is available at any hour of the day or night. The majority of services are provided at no cost. The Health Center does not bill insurance for services involving a charge. The cost is billed to the student’s Whitman account and the student is resposible for submitting receipts to their insurance company.

In order for us to provide the most effective health service, it is essential that you comply with the following health center requirements and return the enclosed health forms to us by August 10th.

• Medical history to be completed by the student
• Physical exam record completed by a licensed M.D., D.O., P.A., or N.P.
• Tuberculosis screening (please read page four of enclosed student health information record). Health care provider must complete and sign the form.
• An Immunization record must be completed by the student.
1. Td (tetanus/diphtheria) booster within the past 10 years
2. Two (2) doses of measles vaccine on recommended schedule
• For students under the age of 18 at the time of entrance, a signed parental treatment authorization (on page one of the enclosed forms) is required.
• Proof of health insurance coverage or purchase of the College health insurance policy

YOU CANNOT FINALIZE YOUR REGISTRATION FOR CLASSES UNTIL ALL OF THE ABOVE CRITERIA HAVE BEEN MET.
